The story follows (Alain Moussi), a former MMA fighter and Muay Thai champion. After defeating the brutal Tong Po in the previous film, Kurt is now married and living in Thailand. However, his past catches up with him when he is kidnapped and taken to a secret underground prison in Bangkok.

The final 15-minute brawl between Alain Mignolet and 6'8" Brian Shaw is a masterclass in David vs. Goliath choreography. Jean-Claude Van Damme delivers a surprisingly touching cameo, and Mike Tyson is genuinely terrifying as a cold-blooded promoter.
